care provider for my mother

We will be hiring a in home care provider for my mother who is terminal with cancer. I would like to know what legal forms or waivers we should obtain to protect not only my mother,but any claims that may be filed against us by the care providor. Any help would be a blessing.Thanks
p.s. My home state is Ohio
