Change of Guardianship


What is the name of your state? GA

I am married and have two daughters, 10 and 12, from a previous marriage. My ex-husband offered to terminate his rights in order for my husband to adopt the children, however the children have decided that they do not want to be adopted. (This is a very civil situation where I have a good relationship with he and his family.) He is in the Navy and lives in Hawaii. He is here for a few days, and this is the first time he has seen the children in over 3 1/2 years. While he is here, he has offered to sign whatever necessary to give my husband "legal guardianship" so that he can make medical decisions, etc. How do we go about this? Can we just write something up and have it notarized? We obviously want to respect the children's wishes, however, he is hard to reach. If there were an emergency it would be better if my husband could make legal decisions regarding the girls.
