changing baby's name

i am an unwed mom in utah. my baby's father's name is on ner birth certificate, and she has his last name. now he wants nothing to do with her. can i legally change her last name to mine? if so, how do i go about it?

look on back of birth cert. there is a phone number call it and they will tell you how to change it
