What is the name of your state (only U.S. law)? WI

We just got married and my wife wants to have my last name. BUT also to move her maiden name to her middle name which is the custom in the Philippines. She is here on a K1 visa.

What is the process? Do we change it at SS office and also ammend her passport to the new middle and last name?

"AFTER THE BIG DAY:

1. Change your name and/or address on your Social Security records. This can be done free of charge at your local Social Security Office after you are married.

2. Change your name and/or address on your driver’s license at the Motor Vehicle Department. This change should be made within 10 days of the marriage.

3. Contact the Post Office to change your name and address. Also notify those people and businesses that send mail to you.

4. Make the necessary name and address changes on your insurance papers and medical records, as well as on all other records and miscellaneous documents."

You will need a state-certified copy of the marriage certificate to make these changes.
