Ct. Guardianship?

An odd situation I'm sure some will think, but my problem is my sixteen year old daughter would prefer to live with my recently widowed mother and I have no issues with this happening, I believe it would be good for them both. What legal steps should we take and how and such??? Please help !!! ty! IBME426
