What is the name of your state (only U.S. law)? Pennsylvania
I need to know and could not find out online anywhere, if an executor passes away, does their spouse take over that title?

I would think the court would have to approve that, or appoint a new executor.

No. The spouse of a person is not the person. The court appointed the executor and will have to appoint a successor.
