Granting my wife joint ownership of my brokerage account

What is the name of your state (only U.S. law)? Virginia

I need to keep my existing individual brokerage account (without closing it and opening a joint account) -- due to trader tax issues and historical record keeping reasons. Can someone please tell me what legal document that I can draft to give my wife half the ownership of this individual account (effectively joint tenancy)?
