Guardian changed my sons name said my whereabouts were unknown when she knew

What is the name of your state? California

What can I do, legally, if my sons grandmother who has guardianship of him, changed his last name and did not notify me. She put "whereabouts unknown" on the paperwork and she knows my address it has been the same for 5 years. Is there something I can do about her deliberatly putting false information on a court document so I wouldn't be sent anything letting me know what she was doing? She attempted to change his name before and I went to court and voiced my objection and the court denied her petition. There has to be something that can be done she can't just ge away with this.
