guardianship

What is the name of your state? Minnesota

I am a single mother. The father of the baby is not on the Birth Cert. and he does not support the child (though he claims to love her and wants her). He lives most of the year in Florida. I am wondering about how to set up a guardian for my daughter in the event something happens to me. I do not want him to be the one to make decisions for her welfare. How do I establish a guardianship, say with my mother as guardian or my brother?
