Guardianship of future grandchild?

Unfortunately, my soon to be 15 year old daughter is going to have a baby. She is currently 32 weeks pregnant, and we just had a close call when she went into pre-term labor. It looks like they have stopped the labor but I can't put this off any longer.

I haven't been able to get my attorney to return my phone calls (time for a new attorney I guess).

My insurance will cover my daughter, but not the baby. I was told by my HR department that I need legal custody or guardianship to be covered by insurance.

My questions are:

1. Is guardianship the best way to go?
2. How difficult is this to get?
3. Can this be done before the baby is born?
4. How much should I expect this to cost?

We are residents of Illinois.

I'm also curious as to if the boys parents are obligated to any child support. (He is 15).
