Guardianship of Grandchild

I live in Texas and our daughter wants us to be the guardian for her daughter for medical insurance purposes. We are wanting to help her. My insurance will cover the grandchild if I am the legal guardian. Our daughter and grandchild both live with us currently and have for the past two years.

Can we do this without an attorney? What is the process?
