What is the name of your state? Colorado

Question:
If you are not married and accept health insurance from your partner of the opposite sex's employer, what are the requirements and implications?
Specifically are you required to sign a common law affidavidt with all that entails, or is a lesser domestic partnership sufficient or nothing, you just live together and can accept insurance? If later you wish to split up are you bound to death or divorce under either of these arrangements or can you simply leave?

Some companies offer insurance to "domestic partners" without there being any requirement that you be "official" in any way. If the company doesn't require you to be offical in any way, then no, it does not create any kind of common law marriage. If the company requires you to be official in some way, then that may be a different story.
