Is Marriage Legal if lied about being divorced?

What is the name of your state? This is M. from Washington. When filling out marriage license in 1974 in the state of Nevada there was a question that asked if you have ever been married before. I asked my husband what I should write and he said to write no. I was married when I was 17 and legally divorced. Could I have problems in the future with our marriage being legal? We have filed a community property and a will with the courthouse. We are also best friends but I threw away my divorce decree from the first marriage, do not remember what date I was divorced and if we need to apply for marriage license in Washington do not know how I would proceed. How would I explain my last name being the same as my husbands?
