Minor name change?

What is the name of your state? California

I currently live in California, however the biological father of my two sons lives in Colorado, and our court orders all remain active in Arapahoe County, CO. I am making a trip back to Colorado in August and was hoping to do a legal last name change for my two sons. The father will consent to the name change, so my question is how quickly and easily can I get it done with consent from the other parent? I will be in CO for approximately 12 days. Does their father have to appear at a hearing with me, or does he just sign a consent, and will there be a hearing or do I just file the paperwork?
