What is the name of your state? Maryland
I pay child support for my daughter who is 12 years old. I also have visitation with her. I always knew that she had my ex-girlfriend's maiden name but now I have found out that my name is not on her birth certificate. How can I have my name added to her birth certificate and can I have her last name changed to mine? |

| | | Talking to her mother is the first option. See how she feels about it. You can't change her last name without the approval of her mother, or order of a judge.
If you had a paternity test through the court, you can contact the office of vital statistics, get the papers to add your name and mail them to the state. |
