Name Change

What is the name of your state? Maryland

I pay child support for my daughter who is 12 years old. I also have visitation with her. I always knew that she had my ex-girlfriend's maiden name but now I have found out that my name is not on her birth certificate. How can I have my name added to her birth certificate and can I have her last name changed to mine?

Talking to her mother is the first option. See how she feels about it. You can't change her last name without the approval of her mother, or order of a judge.

If you had a paternity test through the court, you can contact the office of vital statistics, get the papers to add your name and mail them to the state.

How is your daughter likely to feel about having her name changed at 12?
