Name Change - 47 yr. old male


What is the name of your state? Louisiana
My 47 yr. old son wants to change his name. The clerk of court tells me that there is no form to accomplish this. She suggest that I get a sample of a successful request for name change and have my son use it to construct his own request. Would someone give me an example of a properly writen name change request? Also would like to know where I should submit the completed request.

I don't have anything like that for LA. However, IMHO -- a 47 y.o. can do his own paperwork, make his own phone calls, and submit his own stuff. Don'tcha think?
