Name Change for Minor

What is the name of your state? Florida
I am a single never married mother with full custody of my minor child.
The child was born in Georgia.
I named a person on the birth certificate who is not my husband. He however put his name on the birth certificate at the hospital. I want his name removed and my last name given to the child. He refuses to do this. I have a domestic violence case against him and have been granted a protective restraining order. We also went before a judge in Florida, where I now reside, who has extended the restraining order and given me full parental rights without visitation rights.
What procedure do I use, since the man refuses to give written permission to change my child's name? Must she keep the name?
