name change for minor child over 14 yrs wisconsin


What is the name of your state? wisconsin
My "daughter" has recently asked to change her last name to my last name.
My girlfriend, her daughter and myself have been together as a family now for approx 8 years.
her daughter considers me her "real" dad and wishes to change her last name to my last name.
She is a minor over 14 years old. The biological father is not involved, other than what is required by the state of wisconsin in the divorce decree (child support, health ins.) and has no legal rights to visitation, etc.

Since she is over 14 yrs old, can she do this on her own? Does she need the permission of either of her parents?
