petition for delayed record of birth

What is the name of your state (only U.S. law)? Illinois
How do I file a case for petition of delayed record of birth. What's the whole process?

Why would you want to? Are you trying to prevent the father from knowing his child's been born so you can later claim he never visited the child for whom a record of birth didn't exist?

the child was born at home, therefore the state says "not enough info to prove place of birth, go see a judge." I need to know how to file a case since the courthouse is less than helpful. "We aren't allowed to give legal advice."
I've searched Google, but couldn't find the specifics of filing a case. Do I just take the form from the state to the couthouse and pay a filing fee? Does the courthouse have other forms? Do I need to type up my own forms? Do I even need to file a case? It seems like a simple matter, but I have no experience with this sort of thing. Thank you

If you can't figure it out, it might be worth it to consult with an attorney.

How to file a case

What are the specific steps to file a case? Do I need my own forms or will the court house have forms?

Are you planning on apologizing to OP for that accusation? Since clearly she had a valid reason for asking the question....
