What is the name of your state? Illinois

I have been in a relationship for sixteen years (not married) with the same woman and the relationship might be coming to an end. I have promised to take care of all my financial and personal obligations that I promised I would. She wants some kind of legal paper stating that. I do not have a problem doing this but is it something we need to involve a lawyer in or can a document just be written between the two of us and notorized. Would this be enough for to come after me for the money if I failed to honor my obligations.
