updating divorce documents

What is the name of your state? Texas, Montgomery County.
What is the best way for me to make updates to original divorce documents to reflect change of address and phone numbers and such?
Hate to be a pain, but, I don't have legal experience, so please respond as plainly as possible, what steps to take and things to follow up for, if any.
