At witts end

My mom is turning 71, her memory is getting worse and worse, the kids tell her when their going somewhere and she doesn't even remember that they told her where they were going and if you try to tell her that she forgot she just gets mad, she goes to the bank and takes money out and when she gets home she takes it out of her purse and hides it and then can't remember where she put it and she doesn't even remember that she went to the bank, she goes days with bathing, she'll have change in her purse and she will spend hours counting it over and over again, she refuses to go to the doctor, she forgets to pay her bills, we can't get her to do anything, it's gotten to the point that we are considering declaring her incompetent to take care of herself and I would like to know what we need to do and if it's even possible to do so.
