What is the name of your state (only U.S. law)? Indiana
4 years ago my cousin, a 42 year old white female had a hysterectomy and hemorrhaged and had to have blood transfusion, she was given the "wrong blood" and her health began to deteriorate at that time and 5 months ago was given another transfusion and immediately the deterioration of her condition worsened. Who is responsible for this "medical mistake"? We buried her yesterday.

There is no way to determine if a mistake that rose to the level of malpractice did occur, and if it did who committed it, without a complete and thorough review of her medical records for the past 4 years.

A 40-ish year-old woman who requires blood transfusions and is described as having a deteriorating condition has more wrong with her than having experienced an adverse reaction to a blood transfusion administered four years ago.

Multiple blood transfusion can result in reactions similar or equal to that of receiving the wrong blood type in a transfusion. Blood transfusions predispose the patient to an immune system response. Even receiving the correct blood type in a transfusion can lead to an immune system response that is life-threatening.

I am aware of only liver damage and immunse system response problems as the only longterm damages from a blood transfusion and those usually result from multiple transfusions.
