GF charged for a review of old medical data when going to a new doctor

What is the name of your state (only U.S. law)? Michigan

I'm sorry if this doesn't belong here, but I'm not sure where it should go.

My girlfriend moved to Michigan back in August and had been trying to see a specialist. It had taken about 5 months to get an appointment with the specialist, which is on 1/8. She has not had an appointment with the doctor at all or signed anything.

Prior to the appointment, the doctors office had us pay to ship records from the previous doctor to our house, then to them. This was done around middle of December.

Two days ago, we received a bill for reviewing her medical records (~$250 with the insurance deduction. There were 2 line items that were exact duplicates of the same price) for work that was done on 12/15. We were not notified of a charge to review the medical records nor aware of it. The billing on 12/15 is her first of the year under her new employer, which resets the deductibles on January 1st.

Is there a way to determine if these are valid charges? She has tried contacting the Doctors office and they aren't very helpful as they just want it paid.

If you disagree with the insurance companies explanation of benefits or the exercise of their contract with the doctor call them.

We have not received an EOB from the insurance company.

My main question was whether it is legal to charge for a medical service that was not disclosed.
