Incorrect Information in Medical Records Unresponsive Dr.

What is the name of your state (only U.S. law)? MA

I have an issue with information that has been added into my medical record and Dr. conduct.

1. My PCP will only "authorize" a partial medical record release to me after denying THREE Formal requests for FULL records

In the partial records he has the following "notes":

2nd visit Ever!: "Melodrama Continues" (not even sure what this comes from)

Next visit: "Oh By The Way: Now comes the Big Push for pain meds" & "encouraged to take NSAID, APAP, ASA thinks she was advised not to by Dr. X"
This would be AFTER 2 months of intense pain in head, and joints as well as other neurological and systemic issues and AFTER my Neuro told me (and reported to him) to stop taking aspirin/Tylenol/ibuprofen because of possible "rebound pain". Also during this appt. my Health Proxy asked for a Referral to a Pain Management clinic, or either a rheumo or endo and was told "No, it would diffuse her care"

Following Visit:in "slug position, Feigning slurred speech". (not sure how you "feign" slurred speech when you've had documented slurred speech for 4 months)

He has refused to send or GREATLY delayed or only sent partial medical records to 1.Insurance 2. Neuro 3. Headache Specialist (Referral from Neuro) or 4. Myself. Which has resulted in denial of claims and major delays in treatment.

He has consistently refused to acknowledge any referral recommendations from either the ER, the Neuro (Neuro can only refer to certain specialties without his approval) or requests from either myself or my Health Proxy as it would "Diffuse Care".

As an example of his behavior:
I notified him that the headache specialist was reporting that I was Late Acute Mono positive. I personally faxed his office the blood results and got verbal confirmation that it was received from his secretary. Went to my next scheduled appt. the following Tuesday and he said "I'm not even sure what that means so you should ask Dr. H "(which I did two days later). It was also during this appt. that he told me "It's just pain", and then told me "I'll see you in 6 months" and walked out. Two weeks later, his office calls and says "You need to go see Dr. I for the Mono you should have told Him about it at your last appt." to which I said "I did, and he said...". He NOW says he never knew and never received the blood-work results that I faxed,got verbal confirmation of receipt of and discussed at my appt. with him.

I'm NOT interested in "suing" him, but the "notes" that have been added to my permanent files are incorrect and inflammatory. I believe they are now and will in the future negatively effect dr's I see and or treatment decisions made. I've done EVERYTHING that I've been asked to do, including taking medications I was really not comfortable taking. He refuses to do anything at all, and doesn't listen to other medical dr's(nevermind me), doesn't read results or react to them...etc etc.

I'm not sure what courses of action I have that will remove/amend/correct these "statements" in my medical records?

I'm also not sure if his actions(or inactions) should be reported to insurance/medical board/etc.?

or maybe I'm being "melodramatic"?! :confused:

Any thoughts, advice would be greatly appreciated. Thank you in advance.

You can't remove anything from your records. You can add your own statement if you like. I suggest you make a complaint to the HIPAA board about the unfulfilled records requests and get a new PCP that is not an idiot.
