Question

Discharge Upgrade-Supportive Evidence


I intend to submit a DD 293 in the near future. I'm in the process of gathering the relevant supportive evidence.

I was Entry-level Status in BCT. A drill Sergeant violated the Code of Conduct then refused to allow me to practice my rights when it came to certain things.

My Drill Sergeant banged me over the helmet with a brass rod while I was performing the Basic Rifle Marksmanship qualifying test. I thought somebody was shooting at me.

I was scared to request permission to go to UCMJ.

Heck, I was too scared to know what to do.

I was notified that I was being processed for separation. In this notification, I was referred to a psychiatrist.

Here's how it went. When the DI brought the form for me to sign, he told me that we didn't have time for me to read and to just sign it. He didn't hand me any copy of what I signed. After signing, he said, "this is to see if there is something there that we can't see." Then he walked away.

Within 3 days, the Training NCO took me to the appointment, when I got into the session room, I then found out what I was there for.

The records sent from NPRC didn't include anything about a mental status evaluation

This was 9 years ago, I just learned this past month that all documents from this appointment were destroyed 2-3 years after the date of service. I was not notified at all about it.

So now I have the wrong kind of discharge from a discharge process that didn't consider my mental status evaluation report.

That particular mental status evaluation is critical to my upgrade, and I can't seem to locate the Drill Sergeant or the Company Commander.

Does anyone have any ideas?

So far I found only 3 recommendations of evidence to support my Application for discharge review (DD Form 293)...

1. Character References
2. Exemplary Post-Service Conduct
3. Medical Reports

Are character references letters that show good conduct or are they letters that show conduct which supports a diagnosis, and can I get an example?

What exactly is an Exemplary Post-Service Conduct, and can I get an example?

What are some other forms of evidence?

I'm trying to identify the issues of equity and propriety in my case. Can anyone give me a simple method on how I can do this with little or no difficulty?

Please give examples.
