Legal obligation, or lender requirement?

What is the name of your state?In a NJ mortgage (not the note, but the document sent for recording) the words "signed, sealed, and delievered in the presence of" are to be signed off on by whom?

Is this for the borrower, the notary, or a separate witness?

Who requires this signature, the County Clerk, or the lender?

What are the possible reprecussions of leaving this space blank?
