Missing mortgage disclosure document question

North Carolina. We took out an 80/20 home mortgage loan on 11/14/2005 in North Carolina. This week, the lender notified us the Early Truth in Lending Disclosure Statement for the second mortgage was missing from our loan file. They sent us a copy back dated to 11/14/2005 and asked us to sign and return it. Is this a big deal? Should I just sign and return it, or does this have legal ramifications?
