    Name change

What is the name of your state (only U.S. law)? New York

All my documents including social security card, driver's license, all education certificates and work history is in maiden name. Now I did another certification but I have my married name on it. I called the organziation to change my name back to maiden name. They want legal papers of seperation when there is no question of seperation. What kind of paper work they need? Would my social and driver's license be enough??What is the name of your state (only U.S. law)?
