What is the name of your state? texas
what needs to be included in a statemnt by a witness that will provide proof to clear an accused person of a minor crime to make it and offical stament. a witness signature? if so how many witness must see the stament being wrote

If the state has accused someone, the only way he can clear himself is through a trial. Courts and prosecutors don't just dismiss cases when they receive some signed statements from supposed witnesses, notarized or official-looking or not.
