I am currently a licensed RN in MN. For my job I have to apply for a license in WI. One of the forms asks "List all felonies, misdemeanors, and other violations of state or federal law of which you have ever been convicted." I have a petty misdemeanor from back in 2004 for shoplifting/theft. Does that fall into the "other violations" category? I need to know if I have to get all of the court documents to send in with my application. Any feedback would be great!
