What is the name of your state? Michigan my son lives in Texas. My son's identity was stolen to take out a student loan when he was in college. I have to go to a hearing were and my son is out of state. What form can he use to fill out for his identity being stole?:mad:What is the name of your state?

If his personal identifying information was fraudulently used to obtain money, credit, etc., then he needs to make a crime report. He needs to start with the local law enforcement folks and then take it from there.

Most fiduciary institutions will require a report be filed before they will take steps to correct the issue.
