I live in Louisiana and am considering moving to Texas. How do I transfer my SSI and Medicaid to TX.?What is the name of your state (only U.S. law)?

Medicaid is different from state to state. You would have to re-apply for TX Medicaid after you move. It will not be a seamless transition and it could take a few months to get your benefits re-established, and to find new doctors who accept TX medicaid.
