How are the fees calculated?

I’m from New York State

I have third party and workers comp case. I’m going to trial next week and
I spoke to my lawyer today about the amount that was offered, and he show me what was it(which isn’t what I’m looking for) and he said that we can take that anytime we want to, during the trial. He told me also that the trial would cost money and he mentioned that he had to pay thousands of dollars to doctors and other fees.
Are these fees deducted before the lawyers fee or after?

Another thing, I have a workers comp case (which I have to withdraw) and somehow they have to get paid by the third party case, and I thought they should get paid only if I should had received benefits from workers comp.
Is that something that happened often or they’re taking me for a fool?

Also, I have to pay money to an insurance that I used for my injuries other than workers comp.
Is this amount deducted before the lawyers fee or after?

I thank you a lot for your help
