slip and fall

On 30 Aug 04, 2030 hours, when I was leaving
the buffet in the Market Place Buffet, Grand
Casino,Gulfport, MS, and walking back to my
table, I slipped in a puddle of something wet and
fell on my right hip and right elbow hard. The
floor was tile. The head chef
came out and said,"Look there is water on the floor
that caused you to slip and fall." I have a 4x4"
bruise on my right upper leg and a badly sprained
back. I have been in intense pain so I went to the
Hospital today and had
x-rays of my back and hip.Since the accident, I have had pain.
I have a sprained back and deep bruising. I took
digital pictures of the bruises. At the time of
the accident, the casino EMT and Security person
filed a Risk Management report and I made a
voluntary statement. I have a copy of my
statement. I also have a form from the Casino,
which documents and assigns a number to the
Incident Report.
