exceptions to title insurance

Is it customary for a title insurance company to issue basic exceptions to their policy? The two that concern me are 1)someone not currently reflected in the public records claiming ownership and 2)a lien not reflected in the public records. The big fear I have is the "not in the current public record" part. I thought that's what title insurance is all about. If either of these were to occur due to the information not being available today, it sounds like I could lose my house and still owe the bank.
