If a person has the orginal mortgage and three additional mortgages on the same home in Texas, has something wrong been allowed to happen. If so, who is a fault?

THere is nothing wrong with having multiple mortgages on the property.
Who's to blame? Look to the titleholders who allowed the mortgages.
