Name on contract

My boyfriend and I are buying a condo together. We want to own the place 50/50. I'm not sure how to fill out the contract/loan application -
Name1 and Name2, JTWROS
or
Name1,Name2 JTWROS
or
Name1 and Name2
etc.

The contract is different from the loan application. Use a mortgage broker for the loan application and an attorney for the contract.
