What is the name of your state? Maryland
My father has a deed to 4 grave sites in Maryland which he wants to transfer to me. What do I need to do to complete this transaction? Can I do this myself?

Most cemetary deeds are not true warranty deeds, rather they are deeds issued by a public or private authority. Usually you must go through the authority to transfer the "deed" to another person. Check with a local funeral home that conducts burials in that cemetary to see if they can direct you to the proper place.
