There's no such thing as "Quiet Claim" nor a form for it. Perhaps you mean either Quit Claim (a form of deed, forms available at LEGAL FORMS link at the top of the page) or a Quiet Title action (which is a lawsuit not just a form).

Further, most people who come here looking for Quit Claims, are usually ill-advised to execute one.
