Second Mortgage forged signature

My son lives in Orlando, Fl. His wife obtained a second mortgage
on their home and forged his signature. He did not know that she obtained a second mortgage. She moved out of the home
and left him with this loan. When he called the bank about the loan and then obtained the paperwork that she completed, he discovered that she or someone signed his name. What are his
options. Should the bank have required a notary to witness his signature?
