What is the name of your state (only U.S. law)? Massachusetts

My husband passed away last year and I am sole beneficiary. The only the that did not have my name on it is one bank account under $4000. I received a letter stating I need to get this:

affidavit for collection of personal property

Is this something I can print out, or something I need to file with the court? Thanks in advance for any advice.

I am guessing that they are referring to is what is know in MA as informal administration of small estates. It's a bit much to quote, but you can read the provisions at the link below - Section 16 of Chapter 195. You could try the clerks at the probate court to see if they can assist you.
