What is your state .Wi Ok, mom died in CA and left money,about 8,000 in a bank in New York with no beneficiary.There are four dependents,( brothers and sisters0. Do we sign a form 13101 and send it to the bank in New York. And the form has room for three names only,do we add the fourth name.I'm still a bit confused.Can any one help us please.
