What is the name of your state? OH

I just finished writing a book about a unpleasant personal experience I went through with a well-know public figure. I have been told that I can use his name as well as quotes from his books in my book as long as I use a disclaimer. I am seeking your advice. Can this be done, and if so can you provide an example of how the disclaimer should read.
