I have a duriable medical power of attorney
over someone, he is refusing to let me hire care to help care for him,I feel he needs to be in a residential care he is refusing that
also,I don't know what else to do.Can he be
placed somewhere without agreeing or does something have to be done by a court? What is the purpose of the duriable medical POA
if you have to involve the courts?

Good question -- and the answer is to allow you to make medical decisions ONLY WHEN AND IF THE PERSON CAN NOT MAKE THEM FOR HIM OR HER SELF. Your friend can make the decisions and you have no obligation or right to do so.
