what is the procedure to transfer a free and clear house deed from my mother, who is the owner, to my disabled brother?

She can simply deed him the house. However, this will raise gift tax questions and possibly effect your mother's ability to collect
government benefits. Before you make such a move you need to talk to a tax adviser and real estate attorney. Actually an elder care attorney may be your best bet since he may be able to give you some advice on your brother's situation.
