What is the name of your state? PA

Not sure if this is the correct forum for this but I received a notice about a class action suit that I am not a valid part of. They want a notice to opt-out but I cannot find a good example of this.

Can I simply send a brief letter stating I wish to opt-out or are there good samples I can copy from to send this out.
