Findings And Order After Hearing

I submitted "Findings And Order After Hearing" to the court according to our parenting plan but it was rejected because the opposing party must sign it. If the opposing party refuses to sign it how can I get a copy signed by the judge. Are there certain court approved request forms that I need to submit in order to get the judge to send me a copy?
