What is the name of your state? Indiana

Is it legal to offer health insurance at employee rates that graduate with salary, but has a cap that is far below Upper Management salaries?

We have a $50k salary cap for insurance premiums from employee, with breaks below that for lesser salaries and hourly, except Union members who have a contract already in place.

Is this a legal practice in Indiana, Michigan, and Kentucky?

Yes, this is legal and, as a matter of fact, is becoming more common as companies try to control their insurance costs.
