My husband and I have had two children together. When I had our daughter we had decided prior that it was best we not be together, so his name is not on the birth certificate, and she has my last name. He has since been in her life since she was 2 months old (she is now 2 y/o). We now have a 1y/o son together who does have his last name. We are wanting to change her last name to the same. What is required to do this.
